About The Position

Under the supervision of the VP of Integrated Care, the Director is responsible for leading the planning, development, implementation, and management of integrated care services. This role oversees assigned program operations, staffing, budgeting, financial oversight, data collection and reporting, and quality improvement efforts. The Director ensures that all activities align with organizational goals, funder requirements, and applicable regulations. Integrated Care Models May Include: • Massachusetts One Care Plan/Senior Care Options Plan (for dually eligible Medicare/Medicaid individuals) • Massachusetts Community Partners Program • Other integrated funding and service delivery models as assigned Summary.

Requirements

  • Bachelor’s degree strongly preferred. Master’s degree in human services, psychology, psychiatric rehabilitation, healthcare, or related field of behavioral healthcare preferred.
  • In some cases, experience may be substituted for academic training.
  • Minimum of 8-10 years of experience in health care, behavioral health, or care coordination program management.
  • Demonstrated experience managing budgets, supervising staff, and overseeing multi-stakeholder initiatives.
  • Expertise in serving individuals with complex behavioral health needs, including serious mental illness, co-occurring disorders, and substance use conditions.
  • Experience coordinating care for individuals with complex medical conditions, including chronic disease management and high-risk populations.
  • Understanding of trauma-informed, person-centered, and recovery-oriented approaches to care.
  • Knowledge of integrated care principles, including the intersection of physical health, mental health, and social determinants of health.
  • Knowledge of integrated care systems, value-based care models, and population health management, including the intersection of ambulatory care, hospital care, and community-based resources.
  • Strong analytical and reporting skills, including use of data to guide program decisions and quality improvement.
  • Excellent interpersonal, organizational, and written communication skills.
  • Ability to lead, mentor, and support interdisciplinary teams in a dynamic environment.
  • Strong leadership, organizational, and problem-solving skills.

Responsibilities

  • Works with the VP of Integrated Care to identify, explore, evaluate, and recommend implementation of opportunities in integrated care.
  • Under guidance from the CEO, COO and VP of Integrated Care, develops plans for and directs the start-up and implementation of integrated care projects.
  • Leads the planning, execution, and oversight of integrated care programs, including health homes and other care coordination services as assigned by VP of Integrated Care.
  • Develops and manages program budgets, ensuring fiscal responsibility, alignment with funding requirements, and alignment with organizational goals.
  • Creates and implements operational plans to achieve defined goals, performance metrics, quality standards, and budgetary requirements.
  • Supervises program staff, ensuring accountability for timelines, deliverables, and overall performance.
  • Coordinates internal and external communications and reporting with stakeholders, partners, and funders.
  • Ensures program compliance with federal, state, and local laws and regulations, as well as contractual obligations and generally accepted quality standards.
  • Monitors and reports on key performance indicators, outcomes, and quality benchmarks.
  • Promotes ethical standards and best practices in service delivery and research activities involving program participants.
  • Collaborates with leadership to create and support strategic initiatives and continuous improvement.
  • Coordinates educational and training activities that promote appropriate and effective care for individuals served by integrated care programs, including implementation of evidence-based practices as applicable.
  • Hires, directs, and manages project and program staff assigned by VP of Integrated Care, driving strong employee performance and engagement, and supporting employee development.
  • Maintains a constructive work environment and effective communication with other employees and managers.
  • Directs or participates in contract negotiations and development of projects/proposals for funding to continue or expand services.
  • Prepares and presents written and oral project summaries for internal and external events and conferences.
  • Maintains appropriate written and oral communication.
  • Actively participates in the evaluation of own performance.
  • Communicates a positive image as a representative of Vinfen and the project.
  • Performs other duties as assigned.
