About The Position

Martin's Point Health Care is seeking a Director of Medical Practice - NP/PA (DMP) who will provide primary care services to patients and lead practice site clinicians. The DMP will collaborate with the care team and operations leadership to ensure best-in-class medical service. This role involves managing a patient panel, diagnosing and treating diseases, supporting call coverage, and managing clinician performance related to quality of care, patient access, experience, and value-based care. The DMP will serve as the clinical dyad partner to the Director of Practice Operations (DPO) and foster an engaging clinical culture that embodies Martin's Point values of self-care, continuous learning, mutual support, and enjoyment.

Requirements

  • Master's or doctoral degree from an accredited Nurse Practitioner program or Master's degree from an accredited Physician Assistant (Physician Associate) program.
  • Board certification as NP or PA certification.
  • Active and unrestricted licenses in both ME and NH.
  • DEA license.
  • Active BLS or ACLS Certification.
  • 3+ years of direct patient care experience required.
  • Demonstrated experience working with Clinicians to deploy best clinical practices required.

Responsibilities

  • Ensures outstanding access to high-quality, patient-centric care and education.
  • Provides support and guidance to site clinicians on clinical and business performance, including budget, scheduling, and productivity.
  • Provides direct oversight and guidance to site clinicians on ADDC, value-based care, and quality performance.
  • Implements and supports clinical operations improvement opportunities in partnership with the Operations team.
  • Assures compliance with standards required by government agencies and MPHC policies and regulations.
  • Owns resolution of escalated clinical concerns.
  • Monitors patient complaints and collaborates with the Director of Practice Operations on clinical incidents.
  • Drives continuous improvement of patient care through ongoing assessment of projects.
  • Actively engages in the recruitment and onboarding of new clinicians.
  • Responsible for clinician performance management, experience, and retention.
  • Conducts regular reviews of patient charts to ensure compliance with standards of care.
  • Oversees chart and case review and defines scope and expectations for clinicians.
  • Provides medical services as directed by MPHC.
  • Maintains excellence of care delivery using MPHC’s leadership framework.
  • Leads Quality Improvement initiatives at their site and in collaboration with DPOs and Delivery System leaders.
  • Supports and drives Population Health initiatives and VBC contract goals.
  • Leads clinician monthly meetings in conjunction with the DPO.
  • Participates in daily huddles and regular team meetings to improve workflows and patient outcomes.
  • Responsible for clinician career and leadership development, coaching, mentoring, and communication.
  • Provides educational oversight of clinician learners within the site.
  • Reviews and discusses clinician and delivery system value-based care performance on a consistent basis.
  • Demonstrates the mission, vision, and core values of the organization.
