About The Position

The Direct Support Program Coordinator provides direct support to individuals served by developing and implementing individualized treatments and programs. The role helps individuals access medical and health-related services, assesses referrals, and participates in service planning with treatment teams and funding sources. The coordinator also supports Mentors and staff through monitoring, consultation, and comprehensive guidance, and helps oversee staff in the absence of a manager or director.

Requirements

• High school diploma or equivalent; a bachelor’s degree may be required by the state.
• Three years of related work experience.
• Valid driver’s license in good standing, and car registration and auto insurance if driving for the company.
• Reliable and responsible attitude, with a compassionate approach.
• Commitment to quality.

Responsibilities

• Assist individuals served in accessing medical and health-related services.
• Assess referred individuals and participate in developing service plans with the treatment team and funding source.
• Implement and contribute to the development, review, and revision of programs and behavior intervention plans.
• Provide monitoring, consultation, and comprehensive support to Mentors and staff regarding their work with individuals supported.
• Assist the manager or director with direction, supervision, and oversight of assigned staff.
• Provide effective leadership and direction to staff in the absence of the manager or director.
