About The Position

DaVita Kidney Care is bringing its industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services. This role involves providing complete ongoing care for each patient to ensure their dialysis treatment stays on track, working in conjunction with dialysis RNs. The position requires providing comprehensive patient care in an independent setting, monitoring patients before, during, and after dialysis treatment, including measuring and recording stats, patient observations, and hemodialysis machine setup. The role is based at our site dialysis den at 3526 Dutchmans Ln, Louisville, KY, 40205, with a need to provide coverage for our location at 326 Country Club Dr, New Albany, IN, 47150. Operations function Monday through Saturday, with scheduled days varying based on market needs.

Requirements

• High school diploma or equivalent
• CCHT Certification required
• Basic computer skills and proficiency in MS Word and Outlook

Responsibilities

• Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
• Work in conjunction with dialysis RNs to provide dialysis care.
• Provide safe, comfortable, and hygienic dialysis treatment.
• Monitor patients before, during, and after dialysis treatment.
• Measure and record stats, patient observations, and hemodialysis machine setup.
• Support other local SNF dens as needed.
