About The Position

DaVita Kidney Care is bringing its industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services. This role involves providing comprehensive, ongoing care for each patient to ensure their dialysis treatment stays on track, working in conjunction with dialysis RNs. The position is based at our site dialysis den at 3526 Dutchmans Ln, Louisville, KY, 40205, with a need to support our location at 326 Country Club Dr, New Albany, IN, 47150. Operations function Monday through Saturday, with scheduled days varying based on market needs. This role requires spending the majority of the day in direct one-on-one patient care to provide safe, comfortable, and hygienic dialysis treatment. It is a complex but fulfilling service line where meeting and/or exceeding clinical outcomes are the goals, and multitasking is an expectation. The role demands providing comprehensive patient care in an independent setting, requiring constant critical thinking and application of core skills to monitor patients before, during, and after dialysis treatment, including measuring and recording stats, patient observations, and hemodialysis machine setup.

Requirements

  • High school diploma or equivalent
  • CCHT Certification required
  • Basic computer skills and proficiency in MS Word and Outlook

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
  • Work in conjunction with dialysis RNs to provide dialysis care.
  • Provide safe, comfortable and hygienic dialysis treatment.
  • Meet and/or exceed clinical outcomes.
  • Multitask to meet the needs of the service, the patients and our SNF partners.
  • Provide comprehensive patient care in an independent setting.
  • Monitor patients before, during and after dialysis treatment.

  • Perform patient observations.
  • Set up hemodialysis machines.
