About The Position

DaVita is looking for a Dialysis Clinical Social Worker Licensed in Ohio, to work with our dialysis patients based out of our Dover Dialysis in Dover, Ohio. This position will cover care of patients at three clinics to include: Dover Dialysis, Millersburg Dialysis, and Wooster Dialysis. Requirements

• Licensed in Ohio
• Meets all state required regulations to practice in the dialysis setting.
• All social workers must hold an LSW licensure within the state you are applying
• Master's degree in Social Work (MSW) required with a specialization in clinical practice.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook as well as functional proficiency with DaVita specific applications within 60 days.
• Empathy and good personal skills.

Responsibilities

• Social workers are responsible for the psychosocial health and wellbeing for our patients so we support those needs, providing interventions as needed and complete mandatory assessments, screenings and education
• Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.
• Support and advocate for patients during their transition to life on dialysis.
• Help patients understand their rights and responsibilities.
• Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
