About The Position

Berry Street is building a modern, tech-enabled suite of diabetes programs designed to deliver high-quality, evidence-based care across prevention, education, and long-term management. Our approach combines clinical rigor with a scalable, technology-forward platform to meet the evolving needs of individuals living with or at risk for diabetes. We are seeking a Diabetes Program Manager to lead the design, development, and quality of Berry Street's diabetes-related programs — with a particular focus on asynchronous program models that can scale through technology-enabled care delivery. This is a part-time, contract role for a licensed clinical professional, with broad ownership across program design, curriculum development, clinical quality, and regulatory compliance. You will work closely with operations, product, and data teams to ensure Berry Street's diabetes programs are effective, compliant, and built to scale. This role is ideal for someone with deep, hands-on experience in diabetes programs — including prevention and education — who is excited about designing async-first program models and shaping how high-quality diabetes care is delivered at scale in a modern digital health environment.

Requirements

  • Active clinical or education credentialing such as CDCES, BC-ADM, RDN, or RN.
  • A CDC-approved National DPP Lifestyle Coach and an accredited DSMES provider.
  • Experience designing or leading virtual diabetes programs, with interest or experience in asynchronous and technology-enabled care models.
  • Strong working knowledge of CDC, ADA, CMS, and other diabetes-related program requirements and standards, ideally gained through serving in a formal program coordination, quality, or leadership role within a recognized diabetes program.
  • Direct, hands-on experience delivering diabetes-focused programs, including prevention, education, or self-management.
  • Prior experience in a program coordination, quality oversight, or management role.

Responsibilities

  • Diabetes program ownership & delivery
  • Own the design and development of Berry Street's asynchronous diabetes program models, including prediabetes & DSME curricula, and new programs aligned with CMS initiatives such as the Medicare Access program.
  • Develop structured async program content, participant-facing materials, and care workflows that maintain clinical rigor while enabling scalable, technology-mediated delivery.
  • Ensure programs are grounded in current evidence-based standards and best practices for diabetes care, prevention, and self-management.
  • Serve as a clinical and operational leader for program development, setting standards for participant engagement, session structure, and care quality across async formats.
  • Collaborate closely with operations, product, data, and engineering teams to translate clinical and regulatory requirements into scalable systems and workflows — particularly for chat-based and async care delivery.
  • Contribute clinical expertise to the development of diabetes-related offerings and enhancements, including defining how coaches and educators operate effectively within async models.
  • Program quality, compliance & accreditation
  • Serve as the designated owner of program quality, integrity, and regulatory readiness across Berry Street's diabetes programs.
  • Ensure ongoing compliance with applicable CDC, ADA, CMS, and other relevant regulatory and accreditation bodies, including formally serving as the CDC Program Coordinator and ADA Quality Coordinator.
  • Act as the primary point of contact for external regulatory and accrediting organizations (e.g., CDC, ADA).
  • Establish, maintain, and continuously improve program documentation, operational workflows, and internal controls that support accreditation, audits, reviews, and renewals.
  • Define required program data elements and oversee outcome tracking, evaluation, and reporting efforts.
  • Drive continuous quality improvement initiatives informed by program and participant-level data.
  • Team leadership & clinical oversight
  • Provide oversight for diabetes program staff, including educators, coaches, and clinicians.
  • Ensure team members meet appropriate licensure, certification, and training requirements.
  • Establish quality assurance processes to maintain consistency, fidelity, and high standards of care delivery across async program formats.
