About The Position

The Diabetes Nurse Educator (RN) coordinates, plans, delivers, and evaluates comprehensive diabetes self-management education (DSME) for patients and families, ensuring care aligns with provider goals and individual needs. The role indicates conducting through assessments, identifying barriers, and developing individualized education and care plans in collaboration with patients and the interdisciplinary team. Responsibilities include providing direct education, promoting self-management, and ensuring patient understanding of treatment plans, medications, and supplies. The RN maintains accurate documentation, evaluates patient progress, and adjusts care plans based on outcomes. The educator supports staff education, participates in community outreach, and maintains current knowledge of diabetes care, technology, and reimbursement guidelines. This role also contributes to ADA standards, program development, and quality improvement initiatives, while demonstrating strong communication, teamwork, and the ability to troubleshoot diabetes technologies such as insulin pumps and continuous monitors.

Requirements

  • Graduate of an accredited school of nursing.
  • Current Registered Nurse licensure in the State of Minnesota.
  • Current driver's license.
  • Strong communication skills with the ability to communicate professionally and effectively with clients, physicians and coworkers (verbal and written).
  • Competent to assist in program development.
  • Leadership and teamwork.
  • Organizational skills.
  • Positive attitude.
  • Work independently with minimal direction.
  • Acquire the ability to problem solve with technology glitches or questions related to pumps and sensors.
  • Working knowledge of current American Diabetes Association goals for client diabetes education and management.
  • Has academic preparation and/or experiential preparation in program management and care of persons with chronic disease.

Nice To Haves

  • Bachelor's degree in nursing.
  • Current Certified Diabetes Educator preferred.
  • If not a Certified Diabetes Educator, must have a minimum of 15 hours of continuing education in diabetes specific, diabetes related, psychosocial, and educational topics per recognition period, awarded from an agency that accredits continuing education programs.
