The Diabetes Nurse Educator (RN) coordinates, plan, delivers, and evaluates comprehensive diabetes self-management education (DSME) for patients and families, ensuring care aligns with provider goals and individual needs. The role indicates conducting through assessments, identifying barriers, and developing individualized education and care plans in collaboration with patients and the interdisciplinary team. Responsibilities include providing direct education, promoting self-management, and ensuring patient understanding of treatment plans, medications, and supplies. The RN maintains accurate documentation, evaluates patient progress, and adjusts care plans based on outcomes. The educator supports staff education, participates in community outreach, and maintains current knowledge of diabetes care, technology, and reimbursement guidelines. This role also contributes to ADA standards, program development, and quality improvement initiatives, while demonstrating strong communication, teamwork, and the ability to troubleshoot diabetes technologies such as insulin pumps and continuous monitors.
