The role of Diabetes Educator provides Diabetes and Nutritional educational services and community resources for inpatients and outpatients. This position maintains a case load of patients and promotes program development.

Requirements

  • Education: Nursing Diploma/Associate's - Nursing/Nutrition/related
  • Experience: <2 years
  • Supervisor Experience: No Experience
  • Licenses & Certifications: RN

Nice To Haves

  • Prior experience with Endocrine patients
  • Prior experience in pediatrics
  • Education: Bachelor's Degree - Nursing
  • Licenses & Certifications: Cert Diabetes Educator

Responsibilities

  • Monitors patients response and modifies plan of care accordingly.
  • Provides accurate documentation as required.
  • Delivers direct patient care by assessing and evaluating patient's physical, psychosocial and emotional needs.
  • Develops and implements individual plans of care in accordance with the established guidelines and standards of nursing care.
  • Educates patients and their families on how to manage their condition.
