About The Position

The Corewell Health Center for Diabetes & Endocrinology provides a wide range of specialized services for patients in West Michigan and beyond. Corewell Health operates over 300 outpatient clinics across Michigan, offering a multitude of care options to the communities served. This role provides quality education, counsel, and support to clients and their significant others regarding diabetes self-care.

Requirements

• Bachelor's Degree Nursing or related healthcare field
• LIC-Registered Nurse (RN) - STATE_MI State of Michigan required or CRT-Registered Dietitian (RD) - CDR Commission on Dietetic Registration required
• CRT-CPR - AHA American Heart Association 90 Days required
• CRT-Diabetes Educator (CDE) - NCBDE National Certification Board for Diabetes Educators 3 Yrs required

Nice To Haves

• Master's Degree
• 3 years of recent, relevant clinical experience including prior teaching experience with adult clientele

Responsibilities

• Provides quality education, counsel, and support to clients and their significant others regarding diabetes self-care.
• Documents the education process, patient information and clinical recommendations according to department/program/regulatory guidelines, and measures outcomes to maximize continuity of care between settings.
• Participates in projects related to area of specialty and the development/revision of procedures/policies, educational materials/guidelines, educational programs.
• Collaborates with and serves as a resource for other health professionals to promote delivery of quality services and patient education within the institution as well as the community.
