Requirements

  • Bachelor’s degree in social work or related discipline (i.e. Health Sciences)
  • Minimum three years directly related front line experience
  • Experience working with individuals who have developmental disabilities is essential
  • Familiarity with the local service system and its providers is required
  • The successful applicant will be required to provide a police clearance that meets agency standards
  • A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.

  • Experience in assisting people to develop independent living skills and plan for more independent living is an asset
  • Language skills and an understanding of cultural diversity are considered assets

Responsibilities

  • Provision of assistance and support for adults who have a developmental disability and their families with a focus on transitioning to more independent living situations.
  • Support individuals who would like to live more independently in their community, especially those who live with caregivers who are aging.
  • Support in navigating the service system and exploring all available community service options.
  • Assessment of needs, development and coordination of service plans, referral support and skill development.
  • Provide resources and support through community connections, the use of the DSO Housing Toolkit and by sharing information regarding community specific housing programs.
  • Help individuals connect to local community resources and agencies including working with local municipal housing programs.
