Requirements

Bachelor’s degree in social work or related discipline (i.e. Health Sciences)
Minimum three years directly related front line experience
Experience working with individuals who have developmental disabilities is essential
Familiarity with the local service system and its providers is required
The successful applicant will be required to provide a police clearance that meets agency standards
A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.

Responsibilities

Provision of assistance and support for adults who have a developmental disability and their families with a focus on transitioning to more independent living situations.
Support individuals who would like to live more independently in their community, especially those who live with caregivers who are aging.
Support in navigating the service system and exploring all available community service options.
Assessment of needs, development and coordination of service plans, referral support and skill development.
Provide resources and support through community connections, the use of the DSO Housing Toolkit and by sharing information regarding community specific housing programs.
Help individuals connect to local community resources and agencies including working with local municipal housing programs.
