Requirements

  • Bachelor’s degree in social work or related discipline (i.e. Health Sciences)
  • Minimum three years directly related front line experience
  • Experience working with individuals who have developmental disabilities is essential
  • Crisis prevention and intervention experience
  • The ability to demonstrate sound clinical judgment
  • Familiarity with the local service system and its providers is required
  • Strong analytical, problem solving, conceptual, and facilitation skills
  • Excellent oral and written communication skills
  • Ability to use a variety of software tools, including Microsoft, Data Base, Internet and Email
  • Ability to maintain strict confidentiality of Agency information
  • Travel within the geographic area of York Region
  • A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.
  • Applicants must reside within a reasonable daily commuting distance of the office and/or assigned service area and be able to attend the office as required for operational needs.

  • Experience in assisting people to develop independent living skills and plan for more independent living is an asset
  • Second language is considered an asset
  • An understanding of cultural diversity is considered an asset

Responsibilities

  • Providing intensive case management, advocacy, and support services to individuals aged six and older with developmental disabilities and/or dual diagnosis (developmental disability and mental health).
  • Supporting children, adults, and their families through a person-centered approach to service delivery.
  • Assisting clients in navigating the developmental services system, identifying and accessing appropriate community resources, and exploring available support options.
  • Conducting needs assessments.
  • Developing and coordinating individualized service plans.
  • Facilitating referrals.
  • Supporting skill development to enhance independence, community participation, and overall quality of life.
