About The Position

Yakima Neighborhood Health Services (YNHS) is seeking a Supportive Housing Services Case Manager to provide case management support to homeless individuals. The role involves identifying and outreaching to homeless persons, assessing their needs, developing service plans, and connecting them with resources to stabilize housing and improve self-sufficiency. This position requires working with various agencies, providing assistance with basic needs and entitlements, crisis intervention, and advocating on behalf of clients. Maintaining confidentiality and participating in quality assurance activities are also key aspects of the role.

Requirements

  • Associate degree in behavioral or health sciences or related field and two years closely related work experience.
  • Qualifying work experience may be substituted year for year for education.
  • Knowledge of effective case management practices.
  • Knowledge of government benefits and community resources.
  • Experience in effective social service delivery.
  • Knowledge of addiction and mental health pathologies.
  • Proficiency in computer operating systems.
  • Good attention to detail.
  • Good written and verbal communication skills.
  • Candidate has not been sanctioned or excluded from participation in federal or state healthcare programs by a federal or state law enforcement, regulatory, or licensing agency.
  • Ability to pass a pre-employment background check.

Responsibilities

  • Identification and outreach to homeless persons to determine who is in need of services.
  • Provides case management support, including scheduled and unscheduled home visits.
  • Assessment of need to determine client limitations and strengths.
  • Work with agencies/organizations to maximize clients’ abilities to stabilize housing and improve self-sufficiency.
  • Develop service plan with clients to set goals (including self- management goals) and action steps toward self-sufficiency.
  • Provide assistance with securing food, shelter, clothing and referrals to other supportive services.
  • Provide assistance in accessing entitlements and completing forms and applications.
  • Crisis intervention.
  • Advocate on behalf of clients.
  • Maintaining confidentiality of client interactions and medical records.
  • Participate in staff in-service meetings.
  • Participate in quality assurance activities and peer review as requested.
  • Maintain organized, clean, efficient and confidential work area.
  • Represent YNHS in local meetings as requested.
  • Perform related work and other tasks as requested.
