Yakima Neighborhood Health Services (YNHS) is seeking a Supportive Housing Services Case Manager to join their team. Established in 1975, YNHS is a non-profit organization dedicated to improving the quality of life and equity in their communities by providing accessible and integrated health and social services, ending homelessness, and offering unique learning opportunities for students of health professions. The Case Manager will be responsible for identifying and outreaching to homeless individuals, providing case management support, assessing client needs, developing service plans, and advocating on behalf of clients. This role requires knowledge of case management practices, government benefits, community resources, addiction, and mental health pathologies. The ideal candidate will have an Associate degree in behavioral or health sciences or a related field, with two years of closely related work experience, or equivalent qualifying work experience. Proficiency in computer operating systems and strong attention to detail, written, and verbal communication skills are essential. Bilingual Spanish/English is desirable. Candidates must not have been sanctioned or excluded from federal or state healthcare programs and must be able to pass a pre-employment background check.
