Dementia Outreach Specialist

About The Position

Serve as a liaison for CICOA and various dementia-related programs and initiatives. This role involves coordinating and delivering training sessions, managing volunteers and data, expanding community awareness of dementia programming, and participating in relevant coalitions and outreach events. The specialist will also focus on increasing awareness and access to clinical treatments and support services for individuals with dementia and their care partners.

Requirements

  • Bachelor’s and/or Master’s degree from accredited college or university in social work, gerontology, counseling, psychology, nursing, or public health; or a RN Licensure with one year experience in Human Services; or Bachelor’s degree in any field with an additional certification related to dementia studies

Responsibilities

  • Serve as a point of contact for Dementia Friends Indiana (DFI) and the Dementia Friendly Indianapolis (DFIndy).
  • Coordinate and deliver DFI Information Sessions within CICOA’s service area.
  • Serve as State Lead for Dementia Friends Indiana.
  • Manage the State Dementia Champions, including collection of monthly data, leading quarterly meetings, and communicating session updates.
  • Coordinate and deliver Dementia Friendly Business Trainings within CICOA’s service area.
  • Assist in managing DFI volunteers by facilitating training for new volunteers and ongoing meetings and educational opportunities for established volunteers.
  • Support new and innovative DFI program and partnership initiatives.
  • Expand community awareness of existing dementia programming (Memory Cafés, Living Well with Dementia, etc.).
  • Assist in managing data collection, reporting, and grant provisions.
  • Ensure quality assurance of DFI and brand fidelity with DFI Champions and community partners.
  • Participate in dementia-related coalitions, such as the Indiana Dementia Strategic Plan, IAAAA Dementia Outreach, Indiana Dementia Care Specialist and Dementia Friendly Indiana Communities.
  • Attend conferences, symposiums, and other continuing education opportunities.
  • Attend outreach events when requested and available, promoting DFI, DFIndy, and CICOA programs and materials.
  • Coordinate meetings with potential and existing community partners (including health care providers, health maintenance organizations, and other community-based organizations) to share programs and grow participation and awareness.
  • Increase awareness of and access to clinical treatment, services, and other supports that improve population health outcomes, such as dementia screening, early intervention, clinical access, care management, dementia care coaching, and support of health-related social needs for people with dementia and their care partners.
  • Perform other duties as assigned.
