About The Position

Join High Plains Mental Health Center as a Crisis Case Manager at one of Kansas’ first Crisis Intervention Centers. This groundbreaking facility will play a vital role in transforming mental health care across northwest Kansas by easing the burden on local hospitals and law enforcement, shortening inpatient waitlists, and providing local access to critical mental health care. This is a 24/7 inpatient facility offering multiple shifts. As a Case Manager, you will guide individuals through challenging moments, working closely with a multidisciplinary team to ensure clients receive the care, resources, and support they need during their stay and after discharge.

Requirements

  • Experience counts! We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college.
  • Must be at least 21 years old.
  • Ability to handle high-pressure situations and respond to emergencies.
  • Must pass KBI and KDADS background checks.
  • Must pass drug test.
  • Maintain valid driver’s license and insurable driving record (no substance-related incidents within 5 years).
  • Must meet all health screenings, TB testing, and physical clearance requirements.

Nice To Haves

  • Experience in mental health, crisis intervention, or substance abuse treatment preferred.

Responsibilities

  • Coordinate admissions, transfers, and discharge planning in collaboration with the Clinical Director and care team.
  • Serve as a key liaison between clients, families, legal reps, and community service providers.
  • Develop and implement person-centered treatment and discharge plans focused on recovery, stability, and long-term success.
  • Identify appropriate services or placements following discharge and ensure timely, effective referrals (housing, outpatient care, substance use treatment, etc.).
  • Advocate for clients’ rights and ensure their voice is heard throughout their care journey.
  • Maintain up-to-date and accurate documentation of all case coordination efforts in compliance with HIPAA and KDADS regulations.
  • Work to remove common barriers to discharge such as lack of housing, transportation, or insurance.
  • Support the development of client-centered tools like WRAP (Wellness Recovery Action Plans).
  • Complete all required training in trauma-informed care, de-escalation, crisis response, and more.
