Requirements

  • Associate’s or Bachelor’s degree in healthcare, behavioral health, human services, or related field preferred.
  • 1–2 years of experience in case management, care coordination, or behavioral health services.
  • Level 1 Fingerprint Clearance Card (or ability to obtain upon hire
  • Strong communication, time management, and organizational skills.
  • Familiarity with EHR systems and HIPAA compliance.

Nice To Haves

  • Bilingual (English/Spanish) is a plus.
  • Candidates with equivalent experience in a behavioral health, social work, or medical office setting are encouraged to apply.

Responsibilities

  • Coordinate discharge planning and ongoing care for patients under psychiatric medication management.
  • Schedule follow-up appointments with outpatient psychiatric providers and therapists.
  • Support patient access to prescriptions, transportation, insurance coverage, and social services.
  • Communicate with pharmacies, families, guardians, and care teams to ensure continuity of care.
  • Document case management activities in the Electronic Health Record (EHR) system.
  • Participate in interdisciplinary team meetings and contribute to holistic care planning.
