About The Position

Unity House of Troy is seeking a compassionate individual for a full-time Case Manager position for the Crisis Residence. The schedule is Sunday-Thursday 2pm-10pm. Unity House is a Rensselaer County-based human service agency that provides a wide range of services to meet the otherwise unmet needs of people in our community who are hurting and struggling. We assist those who are living in poverty, adults living with mental illness or HIV/AIDS, victims of domestic violence, and children with developmental delays. Requirements

  • Valid, clean New York State driver’s license required
  • Must have reliable transportation
  • High School Diploma or GED required
  • 1–3 years of experience working with individuals with serious mental illness and/or substance use disorders (in lieu of a degree)
  • Ability to work independently and collaboratively within a team
  • Strong verbal and written communication skills
  • Basic computer proficiency
  • Relevant certifications (if applicable) must be provided

Nice To Haves

  • Degree preferred but not required
  • Lived experience with mental health, behavioral health, or substance use challenges (or as a caregiver/parent of someone with these experiences)

Responsibilities

  • Provide daily crisis intervention and case management services to program participants
  • Offer consistent encouragement, guidance, and positive reinforcement
  • Assist participants in developing Wellness Recovery Action Plans (WRAP) and Safety Plans
  • Provide accompaniment, advocacy, and emotional support as needed
  • Support admissions and discharge planning processes
  • Help individuals build and improve daily living and independent living skills
  • Connect participants with community resources and referrals aligned with their goals
  • Participate in a rotating on-call schedule
  • Foster a supportive environment that promotes inclusion, respect, and belonging
