The Coordinator of START Program is responsible for providing direct service delivery, care coordination, monitoring client progress, client engagement, and ensuring documentation and compliance with agency policies and START standards. This role involves completing various assessments and documentation, maintaining a caseload, providing outreach, coordinating support meetings, and collaborating with individuals, families, providers, and cross-system partners. The coordinator will use START tools to assess needs and plan interventions, monitor progress, and provide rotational on-call crisis support. They will also engage individuals and families respectfully, provide education and consultation, and develop materials for clinical education meetings. Accurate and timely documentation using MHA systems is required, along with maintaining confidentiality and complying with all relevant requirements. Successful completion of START Coordinator training within a specified timeframe is also a requirement.
