About The Position

Cheer Health Care is seeking a compassionate Registered Nurse to join its home care team and provide one-on-one, person-centered support for older adults in their own residences. The role centers on relationship-based care and working closely with an interdisciplinary team to design, manage, and adapt individualized treatment plans. Nurses maintain transparent, ongoing communication with patients, families, and peer clinicians. The position offers 12-hour day and night (“NOC”) shifts, as well as short shifts of 2–4 hours, in La Jolla and North San Diego County. The hourly pay rate is $45–$80, based on experience, with potential for a team bonus.

Requirements

Current, unrestricted California registered nurse license.
Three years of experience as a registered nurse.
Proof of a negative TB result.
Physical screening documentation from within the last year.
Active California driver’s license and car insurance.
Cell phone with internet access.

Responsibilities

Conduct comprehensive physical assessments and evaluate clients’ ongoing needs.
Develop personalized care strategies and establish achievable health milestones.
Provide proactive chronic disease management to help reduce hospital readmissions.
Administer wound treatments, perform dressing changes, and provide post-operative recovery care as required.
Provide health education and coaching to clients and their primary caregivers.
Promote health maintenance and independent daily living.
Work with an interdisciplinary care team to design, manage, and adapt individualized treatment plans.
Maintain transparent, ongoing communication with patients, families, and peer clinicians.
