About The Position

Cheer Health Care is hiring a compassionate Registered Nurse to provide one-on-one, person-centered home care to older adults in their residences. The role emphasizes relationship-centered care and close collaboration with an interdisciplinary team to design, manage, and adapt individualized treatment plans. The nurse maintains transparent, ongoing communication with patients, families, and peer clinicians. Shifts include 12-hour day or night shifts and short shifts of 2–4 hours in La Jolla and North San Diego County.

Requirements

Current, unrestricted California Registered Nurse license.
Three years of experience as a registered nurse.
Proof of a negative TB result.
Physical screening documentation from within the last year.
Active California driver's license and car insurance.
Cell phone with internet access.

Home Health experience preferred.

Responsibilities

Conduct comprehensive physical assessments and evaluate clients’ ongoing care requirements.
Develop personalized care strategies and establish achievable health milestones.
Provide proactive chronic disease management to help reduce hospital readmissions.
Administer wound treatments, perform dressing changes, and provide post-operative recovery care as needed.
Educate and coach clients and their primary caregivers on health-related matters.
Promote health maintenance and independent daily living.
Collaborate with an interdisciplinary care team and maintain ongoing communication with patients, families, and peer clinicians.
