About The Position

Cheer Health Care is hiring a compassionate Registered Nurse to provide one-on-one, person-centered home care for older adults in their residences. The nurse works alongside an interdisciplinary care team to design, manage, and adapt individualized treatment plans, while maintaining continuous communication with patients, families, and peer clinicians. Shifts include 12-hour day or night shifts and short shifts of 2–4 hours. The role serves La Jolla and North San Diego County. Hourly pay is $45–$80, based on experience, with potential for a team bonus.

Requirements

Current, unrestricted California registered nurse license.
Three years of experience as a registered nurse.
Proof of a negative TB result.
Physical screening documentation from within the last year.
Active California driver’s license and car insurance.
Cell phone with internet access.

Responsibilities

Conduct comprehensive physical assessments and evaluate clients’ ongoing needs.
Develop personalized care plans and establish achievable health milestones.
Manage chronic conditions proactively to reduce hospital readmissions.
Provide wound treatments, dressing changes, and post-operative recovery care as required.
Educate and coach clients and their primary caregivers about health.
Promote health maintenance and independent daily living.
Communicate continuously with patients, families, and peer clinicians.
Collaborate with an interdisciplinary care team to design, manage, and adapt individualized treatment plans.
