About The Position

Halcyon Home is seeking an energetic, detail-oriented, compassionate, and highly motivated Concierge Home Care RN for its private-pay nursing services. The role provides personalized nursing care in clients’ homes, assisted living communities, and independent living residences, with a focus on listening to clients and supporting their individual needs and preferences. The nurse manages medications, oversees RN delegation, and provides clinical guidance to caregivers. The position also involves close collaboration with caregiver teams and marketing liaisons, supporting new client transitions, and building trusted relationships with clients, families, caregivers, and residential community partners. The RN must respond promptly to changing client needs while remaining calm, attentive, and compassionate. The listed pay range is $35–$50 per hour.

Requirements

• Current RN license in good standing with the state nursing board.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.
• Strong clinical judgment and commitment to safe, personalized care.

Nice To Haves

• Knowledge of or experience using Homecare Homebase EMR.
• Experience in home health or a related healthcare setting.

Responsibilities

• Deliver concierge nursing care tailored to each client’s needs and preferences.
• Manage and oversee medications to promote safety and accuracy.
• Oversee RN delegation and ensure caregivers are trained, supported, and equipped to provide quality care.
• Collaborate with caregiver teams and provide clinical guidance, leadership, and support.
• Partner with marketing liaisons to build relationships, support new client transitions, and ensure a positive client experience.
• Respond to changing client needs promptly and confidently.
• Build trusted relationships with clients, families, caregivers, and residential community partners.
