About The Position

The Home Care Concierge provides compassionate in-home support to help participants remain safe, independent, and engaged in daily life. Support includes companionship, assistance with daily living activities, meal preparation, light housekeeping, medication reminders, transportation, and support during medical appointments. The role may also assist with urgent participant and operational needs as they arise.

This is a full-time role in the Home Care department at Huron Valley PACE in Monroe, Michigan. The schedule is 40 hours per week, Monday through Friday, from 8:30 a.m. to 5:00 p.m. The listed duties are a summary and may not include every duty required; a full job description is provided if selected.

Requirements

• High school diploma or GED.
• Reliable transportation and a valid driver’s license.
• Current CPR certification or ability to obtain it within 90 days of hire.
• At least one year of experience working with frail elderly or long-term care populations.
• Ability to lift 35 pounds without assistance.
• ServSafe Food Handler Certification or ability to obtain it within 90 days of hire.

Responsibilities

• Provide one-on-one in-home support, companionship, and respite care to participants.
• Assist with daily living activities, including light housekeeping, laundry, meal preparation, and medication reminders.
• Engage participants in meaningful activities that promote independence and well-being.
• Provide transportation support and accompany participants to medical appointments as needed.
• Assist with clinical appointment support, including participant preparation and documentation.
• Collaborate with the care team and provide operational support as needed to ensure quality participant care.
• Participate in training and maintain compliance with safety and care standards.
