About The Position

AHRC New York City, a large nonprofit organization supporting neurodivergent children and adults, seeks a Community Support Supervisor. This role involves developing service direction with a focus on personal future planning and community inclusion. The supervisor will foster independence, productivity, and community inclusion of individuals served, while protecting their health, safety, well-being, and civil rights. Key responsibilities include ensuring individualized habilitation plans are consistent with service plans, supporting team efforts, and documenting progress. The role also involves facilitating the development of individual support networks, coordinating outreach to family and community members, and organizing personal planning sessions. Direct support staff training, including orientation, arranging training, maintaining records, and supporting staff development, is also a crucial part of this position. The supervisor will evaluate staff performance annually, develop goals, and monitor progress. They will also assign duties during staff absences and ensure adequate supervision for individuals served. Effective communication with employees and outside agencies for community opportunities, and acting as a liaison between Service Coordination Services and Day Habilitation service, are essential. Participation in case conferences, staff, and parent meetings is expected. Ensuring the safety and health of individuals through monitoring safety issues, coordinating medical support, practicing emergency procedures, and responding to emergencies is paramount. Enhancing staff relations through effective communication, encouraging growth, facilitating teamwork, and employing conflict resolution skills are also key. The role requires ensuring program compliance with New York State Home and Community-Based Waiver Services regulations, agency philosophies, and policies.

Requirements

  • Masters in Business Management or related field.
  • Will accept a single degree or any combination of degree, diplomas or professional credentials determined to be equivalent to a US Masters in Business Management by an accredited credential evaluation service.

Responsibilities

  • Develop direction for service with clear philosophy and expectations toward personal future planning and community inclusion; develops activities to implement this philosophy.
  • Foster independence, productivity and community inclusion of the individuals served while protecting their health, safety, well being and civil rights.
  • Ensure that individualized habilitation plans are consistent with individuals’ service plans and are supported by the team; follows through on each individual’s established outcomes and documentation of progress by the Community Support Professionals.
  • Facilitate and support the development and maintenance of individual support networks through outreach to family members, community members, and professionals and through coordination of personal planning sessions in collaboration with the individual served.
  • Coordinate and participate in direct support staff training and in-services by orienting new staff, arranging for staff to attend training, maintaining training records, and supporting ongoing staff development.
  • Evaluate assigned support staff on an ongoing basis implementing a job performance evaluation annually; develop appropriate goals for staff and follow through on staff progress.
  • Assign staff various duties particularly in the case of staff absences; ensure that there is adequate supervision for the service needs of the individuals served at the program.
  • Facilitate effective communication with employees and outside agencies for ongoing development of community opportunities.
  • Function as liaison between Service Coordination Services and Day Habilitation service.
  • Participate in any special case conference, staff and parent meetings as appropriate.
  • Ensure that individuals supported are safe and living healthy lives by monitoring safety issues, coordinating, monitoring and documenting medical support, practicing appropriate emergency procedures, responding to emergencies and promoting individual rights regarding health and safety issues.
  • Enhance staff relations by using effective communication skills, encouraging growth and self-development, facilitating teamwork, employing conflict resolution skills, and providing adequate support to staff.
  • Ensure that program remains in compliance with New York State Home and Community-Based Waiver Services regulations, agency philosophies, and policies.
