Lutheran Social Services of Southern California (LSSSC) began in December 1944 with a vision to assist families in need. Officially incorporated in 1946 as a 501(c)(3) non-profit social service agency, LSSSC's mission remains true to its earliest beginnings: to be a servant to those in need. Today, LSSSC serves thousands of individuals and families throughout Southern California with over 45 programs/services at 18 locations. The Community Supports (CS) Case Manager I, II, III, under the supervision of the Program Director or Program Manager, is responsible for managing and overseeing all case management activities for assigned programs. This role operates from a mobile wellness unit, offering on-the-go support to clients across Los Angeles and Ventura counties. The case manager will assist individuals and families in navigating resources such as housing support, social services, and other community-based services. The focus is on client-centered care, ensuring timely and accurate service delivery while supporting clients in accessing essential services.
