Requirements

  • Genuinely knows the Kentucky behavioral health and social services landscape, or can credibly build that knowledge fast
  • A relationship-first operator who is comfortable walking into a hospital discharge planning meeting, an FQHC clinic, and a food bank in the same day
  • Understands the realities of serving people with serious mental illness, including the role of housing, food, transportation, and trauma, and doesn't flinch at hard cases
  • Energized by the blank page and is comfortable creating processes where none exist
  • Holds themselves accountable to metrics without losing sight of the relationships behind them
  • 5+ years in community engagement, business development, outreach, or partnerships in healthcare or social services
  • Deep familiarity with Medicaid populations and the services that surround them
  • Comfortable with CRM tools and basic data tracking
  • Based in Kentucky, with the ability to travel across the service area regularly

Nice To Haves

  • Existing relationships in the Kentucky behavioral health, hospital, or community-based organization landscape strongly preferred

Responsibilities

  • Build Constella's referral and partnership network across Kentucky from scratch, including hospital systems, FQHCs, Community Mental Health Centers, crisis stabilization units, and inpatient psychiatric facilities
  • Develop relationships with community-based organizations that touch our members every day: shelters, food banks, housing programs, recovery communities, faith-based organizations, and social service agencies
  • Serve as Constella's primary point of contact for external partners in the market. You're the person they call when they have a member who needs help, and the person who makes sure that warm handoff actually works.
  • Design and run a repeatable partnership motion: target list, outreach, first meeting, pilot, formal agreement, and ongoing relationship management. You use data to build a target list and execute against it diligently
  • Work hand-in-hand with our Community Health Workers and clinical team to turn partnerships into real referral flow and real wraparound support for members
  • Track partnership performance (referrals in, successful engagements, closed-loop feedback to partners) and iterate on what's working
  • Represent Constella at local behavioral health coalitions, provider meetings, and community events. Become a known and trusted presence in the Kentucky behavioral health ecosystem.
  • Partner with the GM and clinical leadership to identify gaps in the local resource landscape and shape how we fill them
