About The Position

Empath Health, a not-for-profit healthcare organization and member of Empath Health, is seeking a compassionate, bilingual Community Outreach Specialist to join its interdisciplinary PACE team in Tampa, Florida. The specialist drives program growth through strategic community engagement, relationship development, and high-quality referral generation. PACE (Program of All-Inclusive Care for the Elderly) is a care model that helps older adults remain safe and independent in their own homes. It emphasizes relationship-based care, long-term participant support, and proactive collaboration among clinicians, with a focus on prevention, wellness, and holistic well-being. The position develops community awareness and referral relationships, guides prospective participants and families through enrollment, and represents the program with professionalism and empathy. It also requires knowledge of the local healthcare market and policies that affect outreach and access, along with the ability to prepare and deliver presentations to referral sources and prospective participants.

Requirements

Bilingual ability is required.
At least two years of experience in health plans or senior-focused programs, with demonstrated success meeting or exceeding sales goals.
Strong customer service orientation.
Ability to work independently with minimal supervision.

Nice To Haves

Bachelor’s degree in marketing or healthcare administration preferred.
At least two years in a healthcare or senior care setting preferred; experience within the service area and outside sales experience are strongly preferred.
Experience with CRM software preferred.

Responsibilities

Build and maintain relationships with community partners, referral sources, senior-serving organizations, and healthcare providers.
Develop and execute outreach strategies to increase awareness of the PACE program.
Generate high-quality referrals and guide prospective participants and families through enrollment.
Represent the PACE program professionally and empathetically, demonstrating understanding of geriatric care needs.
Meet or exceed performance metrics for outreach activity, referral volume, and enrollment growth.
Identify communities and populations of Medicaid, Medicare, Dual Eligible, VA, and Private Pay individuals, and align Empath Life PACE services through referral channels.
Serve as a community brand ambassador to strengthen program visibility and reputation.
Maintain knowledge of the community healthcare market, competition, consumer demands, and governmental policies influencing outreach goals and patient and family access.
Create and deliver effective proposal presentations to referral sources and prospective participants.
