About The Position

The Health Home Outreach Coordinator develops and maintains referral partnerships and supports enrollment growth for HeartShare's Adult Health Home (AHH) and Children's Health Home (CHH) programs. The position conducts community-based outreach, builds relationships with healthcare and social service organizations, engages prospective members, and supports eligible individuals through the enrollment process. The Coordinator represents HeartShare professionally and maintains accurate outreach, referral, and enrollment information. The position may be eligible for performance-based incentives in accordance with the approved Health Home Outreach Coordinator incentive policy.

Requirements

  • High school diploma or GED required
  • Associate degree in human services, social services, healthcare, business, communications, or a related field preferred.
  • Experience in community outreach, care management, healthcare, behavioral health, social services, enrollment, recruitment, or relationship development preferred.
  • Strong relationship-building skills and ability to sustain partnerships with healthcare providers, community organizations, schools, government partners, and internal programs.
  • Persistence, follow-through, and comfort working toward measurable outreach, referral, conversion, and enrollment expectations established through the annual Outreach/Performance Plan.
  • Ability to conduct frequent field-based outreach and community visits.
  • Strong verbal, written, presentation, and interpersonal communication skills.
  • Ability to organize multiple leads, follow-ups, referral sources, and priorities while maintaining accurate documentation.
  • Strong problem-solving skills and ability to develop practical, client-centered solutions.
  • Ability to work independently and collaborate with Health Home leadership, care management teams, and other HeartShare programs.
  • Working knowledge of Microsoft Office and ability to learn required tracking, documentation, and reporting systems.
  • Knowledge of Health Home services, Medicaid populations, behavioral health, social determinants of health, or community resources preferred.

Responsibilities

  • Collaborate with Health Home leadership to develop and maintain referral partnerships that support AHH and CHH enrollment growth.
  • Develop relationships with HHC and other hospitals, health systems, Managed Care Organizations (MCOs), medical and behavioral health providers, schools, community organizations, shelters, foster care and preventive service programs, and internal HeartShare programs.
  • Conduct proactive outreach through field visits, community events, presentations, telephone calls, electronic communication, and other approved engagement methods.
  • Engage prospective members and referral sources, explain Health Home services and eligibility, and support timely completion of the enrollment process.
  • Maintain an active referral and outreach pipeline and consistently follow up with prospective members and referral partners.
  • Track outreach activities, referral sources, lead status, follow-up activity, enrollment progress, and outcomes accurately.
  • Deliver presentations to internal and external stakeholders about Health Home services, eligibility requirements, and referral processes.
  • Identify barriers to referral, enrollment, and engagement and work with program staff to resolve them in a timely, client-centered manner.
  • Monitor outreach and referral trends and adjust strategies with program leadership based on program needs and results.
  • Maintain detailed knowledge of AHH and CHH eligibility criteria, referral requirements, and enrollment processes.
  • Provide accurate and timely referral logs, enrollment information, outreach summaries, and weekly/monthly status reports.
  • Performs other job-related duties as assigned, consistent with the scope, responsibilities, and level of the position, and in compliance with applicable laws, regulations, and organizational policies.
