About The Position

The Community Outreach Coordinator (COC) is responsible for representation of the program in the community and offering services to eligible clients. The COC’s primary duties include calling referred clients to schedule intake assessments, maintaining referral records, responding to requests for HANDS to attend local community events relevant to FCP’s client population, attending community events to obtain program referrals, complete rounds at local hospitals & pediatrician offices with new parents to obtain program referrals, and coordinate/host HANDS’s monthly community baby showers. The position works alongside community partners and other FCP staff to coordinate events, build referral relationships, and maintain avenues for HANDS to be visible within the Louisville community so that program services can be continually promoted. The COC may provide information and referrals about community resources as needed when potential clients decline HANDS services or are ineligible to enroll in HANDS services. The COC’s work schedule varies but is required to be flexible to include some evenings and weekend hours. The COC is responsible for maintaining personal & independent transportation and a valid driver’s license.

Requirements

Requires advanced knowledge of human services including factors precipitating abuse and neglect, childhood development, interviewing skills, and relational skills.
Must have good engagement skills with individuals and families, children and adults, and community partners.
Understands the impact of trauma on clients’ ability to engage in agency programming and case planning.
Ability to communicate effectively with clients/families and community partners.
Good writing and organizational skills.
Knowledge of community resources.
Requires the ability work independently and manage time well (effective time management skills).
Must be able to appropriately respond to the cultural differences present among the organization’s service population, including relating to people with respect for their individuality, and being culturally sensitive and appropriate.
Current CPR certification
Recognizes the effects of secondary trauma and seeks to mitigate its effects through supervision, training and focus on caregiver wellness.

Frequent local travel is required for this position.
Must have own transportation and valid driver’s license.
Prefer Bachelor’s degree in social science, human services, or related field AND one year experience working in community outreach or referral intake/assessment scheduling services.
An equivalent combination of education and experience may be substituted.

Responsibilities

Conducts visits to hospital wards (Labor and Deliver, Ante-partum and NICU) and other medical offices to offer new and expectant parents HANDS services.
Completes referral forms and submits in a timely manner according to program policy.
Contacts clients for the purpose of scheduling assessments for the Assessment staff according to instructions by the AHV Supervisor.
Notifies staff of scheduled visits according to protocols/instructions
Attends community health fairs and events to promote the HANDS program.
May screen parents for eligibility to participate in the HANDS program.
Seeks to attend to the whole person and their environment through a culturally competent trauma-informed approach.
Assists supervisor in program planning and coordination.
Participates in regular individual supervision with supervisor to review outreach efforts and results as well as scheduling status.
Participates in mandatory HANDS core training and other training as required by the Agency.
Completes recordkeeping and data collection and submits in a timely manner, according to program policy.
Documentation reflects appropriate trauma-informed interventions and correspondence.
Direct service staff is required to participate in other administrative programming requirements (such as team meetings, client events. peer review committee, CQI leadership, Wellness committee, Safety Committee, etc.)
Performs other duties as assigned and delegated by supervisor.
