The Community Licensed Practical Nurse (LPN) supplements participants’ nursing care needs as established in the Interdisciplinary Team (IDT) plan of care. Working under the direction of an RN, physician, or nurse practitioner, the LPN collects specific assessment data used by the IDT to plan and evaluate care, and performs duties only within the scope of their authority to practice. The LPN works with participants, families, and interdisciplinary team members to implement the plan of care. Duties include assisting with data collection during clinic visits, communicating findings to medical providers or the team, preparing and administering prescribed medications safely and accurately, explaining medication actions and side effects, and instructing participants and families in safe medication administration at home. The LPN documents medications and care, reports and follows up on medication errors, maintains accurate records of participant functional status, advocates for participants, and works to address concerns and support participant satisfaction. The LPN participates in required annual training and screening, continuing education, nursing research data collection, and review of relevant nursing and PACE literature. The role requires current Michigan LPN licensure, at least one year of experience with a frail or elderly population, completion of standardized competencies before working independently, and medical clearance and up-to-date immunizations before direct participant contact. Work conditions may involve exposure to communicable diseases and physical tasks such as walking, stooping, standing, reaching, bending, and lifting 40 pounds or more.
