Enhabit Home Health & Hospice•Columbia, MS

About The Position

The Community Liaison serves as the primary relationship manager between Enhabit and referral sources, including physicians and medical practices, hospitals, health systems, inpatient facilities, senior living communities, professional associations, and other healthcare organizations and professionals. The role supports referral growth by educating referral sources, patients, families, caregivers, and community partners about company services and program requirements. This support helps people navigate post-acute care transitions, with the goals of improving patient outcomes, supporting care transitions, and promoting exceptional quality and patient and referral-source satisfaction. The position requires a relevant bachelor’s degree or graduation from an approved school of nursing, therapy, or social work; equivalent work experience may substitute for the degree requirement. Candidates need at least two years of relevant healthcare business development, community relations, marketing, or sales experience, as well as demonstrated knowledge of laws and regulatory guidance governing Medicare-certified home health and hospice operations. The role also requires a valid driver’s license, automobile liability insurance, dependable transportation, and strong communication and presentation skills. Clinical candidates must maintain the applicable clinical license. Except for designated full-time remote positions, in-office attendance is considered essential to achieving Enhabit’s objectives. Occasional remote work to accommodate temporary personal needs or during crises does not waive this essential function.
