The Home Health Community Liaison is responsible for maintaining relationships with referral sources such as hospitals, long-term care facilities, rehabilitation centers, and physician practices. This role works closely with agency sales representatives to ensure effective communication and smooth transition of referrals. The liaison efficiently processes referrals according to agency and referring provider policies, and accurately explains Eden Health's program to patients, family members, and other referral sources. They collaborate with agency staff to ensure a safe transition to home for all patients and provide timely documentation of marketing activities. The position involves following up on referrals for evaluation or admission, attending in-services and staff meetings, and completing all documentation accurately and legibly. The liaison must also obtain outstanding orders or documentation as requested and meet or exceed home health and hospice referral and admission goals.
