About The Position
The Community Liaison Home Health serves as the primary relationship manager between Enhabit and referral sources, including physicians and their practices, hospitals, health systems, inpatient facilities, senior living communities, professional associations, and other healthcare organizations and professionals. The role supports referral growth by providing awareness and education about company services and program requirements to referral sources, patients, families, caregivers, and community partners. This helps people navigate post-acute care transitions, with the goal of improving patient outcomes, supporting care transitions, and promoting exceptional quality and patient and referral-source satisfaction. In-office attendance is essential for positions that are not designated full-time remote, to support supervision, teamwork, task completion, and other position objectives. Occasional remote work may be permitted for temporary personal needs or circumstances beyond the company's control. Requirements
- Bachelor’s degree in healthcare administration, marketing, or a related field, or graduation from an approved school of nursing, therapy, or social work; equivalent years of work experience may substitute for the degree requirement.
- At least two years of experience in healthcare business development, community relations, marketing, or sales.
- Demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing Medicare-certified home health and hospice operations.
- Valid driver license in the current state of residence, maintained while employed.
- Automobile liability insurance as required by law, maintained while employed.
- Strong interpersonal, communication, and presentation skills.
- For clinical candidates, a clinical license maintained in accordance with state requirements and applicable board rules.
- Dependable transportation in good working condition and ability to drive safely in all types of weather.
- Oral and written communication skills, fluency in English, and active listening.
- Intermediate technology skills, especially Microsoft Office, including Word and Excel.
Nice To Haves
- Graduation from an approved school of nursing, therapy, or social work is highly preferred.
- Experience in home health, hospice, post-acute care, or related healthcare services is highly preferred.
- Previous experience using electronic referral systems and Homecare Homebase (HCHB).
- Established relationships with physicians, hospitals, senior living communities, or other referral sources.
