About The Position

Community Options, Inc., a national nonprofit agency providing services to individuals with disabilities in 11 states, is hiring a Community Integration Counselor in Feasterville-Trevose, Pennsylvania. The role assists individuals in learning physical, intellectual, emotional, and social life skills. It provides individualized support to help participants manage and overcome challenges associated with living in the community. The position has open availability, including nights and weekends, and starts at $18.60 per hour.

Requirements

High school diploma or GED.
Valid driver’s license and satisfactory driving record.
At least two years of relevant experience supporting individuals with intellectual and developmental disabilities.
Ability to establish a comfortable and supportive relationship with individuals receiving supports.
Successful completion of criminal background, central registry, child abuse registry, and drug-testing checks is required for employment.

Bachelor’s degree preferred.

Responsibilities

Support and encourage individuals to increase their independence.
Meet regularly with waiver participants in their residence, in the community, or in an office setting.
Provide ongoing support for community integration efforts.
Document individual progress toward acquiring community integration.
Help build support networks between individuals and the community.
Administer medications and complete required documentation.
Coordinate leisure and volunteer opportunities.
Monitor individuals’ health and medical needs and promptly report concerns.
Drive individuals to medical appointments and community activities.
Complete program documentation and billable records accurately and on time.
Cooperate with license and department staff during inspections, inquiries, or investigations.
Perform additional tasks and responsibilities as assigned.
