About The Position

A Community Health Worker (CHW) at Oak Street Health bridges patients, their communities, and medical systems to remove barriers and support wellness across all life domains. The CHW advocates for and liaises with patients through proactive in-person and phone outreach based on their care needs, promoting health literacy and access to resources that support healthier lives. The role calls for flexibility, problem-solving, strong communication, and knowledge of the community served. No clinical license is expected, and CHWs work within their scope of practice. CHWs work closely with Medical Social Workers to manage patient care plans, support care-team decision-making, and coordinate clinical and complementary services. They meet patients at patient-centered locations, including Oak Street Health centers, homes, external medical facilities, and community settings. About half of the role is spent outside the center, so reliable transportation is required. The position is full-time, with 40 anticipated weekly hours.
