About The Position

Saint Alphonsus Health System is seeking a PRN Community Health Worker for its mobile health clinic serving rural and underserved communities in Western Idaho and Eastern Oregon. The clinic provides free healthcare and vaccination services at locations such as migrant farms, homeless shelters, and community partner sites. The clinic operates similarly to an urgent care and is staffed by a provider, registered nurse, and medical assistant. The CHW supports marginalized populations, including patients whose first language may not be English, and serves as a liaison among patients, the healthcare system, and community organizations. The role includes screening for health-related social needs, documenting in Epic, connecting patients with local resources, and following up to support their ongoing health and well-being. This as-needed position requires full orientation and training, including in-person classes at Boise Regional Medical Center. The employee is expected to work five mobile clinic shifts per month, with the opportunity to work up to 16 additional hours per week on other CHW programs and projects. Clinics typically operate from 8:00 AM to 5:00 PM and rotate among Mondays, Tuesdays, and Thursdays; occasional Saturday clinics may be offered. Some clinics may run into the evening or on weekends, with pay differentials when available. The position involves travel to clinic sites, setup and breakdown of equipment, and working outdoors in varying weather conditions. Mileage reimbursement is available for travel to and from clinic sites. The PRN position does not qualify for benefits but does qualify for retirement and employee discounts.
