About The Position

The Community Health Worker (CHW) conducts screenings and navigates individuals to community resources and medical homes, when appropriate, to address non-medical drivers of health (NMDOH). This entry-level role supports patients and community members by identifying barriers such as food insecurity, housing instability, and lack of access to care, then facilitating timely connections to services. The CHW builds trust, documents social needs, and coordinates with care teams to help ensure individuals receive comprehensive support. Client engagement is primarily remote, while the position is based on-site at a designated Memorial Hermann campus. The role uses digital tools and contact center-enabled technology to extend reach and improve coordination. Occasional travel to clinical or community settings may be required, and support at other Community Health Network locations may be needed. The position typically reports to a Manager within the Community Health Network and/or the Memorial Hermann Contact Center.
