About The Position

Southern Jersey Family Medical Centers, Inc. is seeking Community Health Workers to improve health outcomes in Atlantic County. The Community Health Worker (CHW) will work as a part of the Community Engagement Team and Program Development Team on the Sickle Cell Disease Pilot Program. The CHW will go out into the community to educate, engage, and work with the community focusing on those with Sickle Cell Disease and Sickle Cell Trait. Local travel is required. This is a grant position scheduled to run from 9/1/2026 - 6/20/2027.

Requirements

  • High school diploma or equivalent
  • Health/social services experience or education
  • Strong verbal and written communication skills
  • Creative approach to problem-solving
  • Computer skills: EPIC experience preferred
  • Valid driver’s license and reliable transportation

  • Prior experience or education in health and/or social services specially related to Sickle Cell Disease and Sickle Cell Trait

Responsibilities

  • Community outreach to promote the Sickle Cell Disease Pilot Program
  • Sickle Cell Disease outreach; includes assisting with case coordination, patient education, and all grant related activities
  • Establish positive, supportive relationships with community partners
  • Working with the community to increase engagement in initiatives, as well as patient recruitment and retention within the health centers
  • Effectively work with our diverse patient populations to address social determinants and identify needs through screenings and assessments
  • Organize and coordinate outreach events
  • Encourage patients to be active, engaged participants in their health
  • Public speaking and presentation skills to diverse groups of individuals, including community partners, meetings, and events
  • Continuously expand knowledge and understanding of community resources, services and programs provided
  • Excellent verbal and written communications skills
  • Ability to work independently with regular support and supervision
  • Other duties as assigned
