About The Position

We are hiring for two Community Health Worker I positions. The role carries out health service duties involving communication, education, and training activities under the supervision of, or after consultation with, one or more health specialists.

Requirements

• An Associate of Arts degree from a community college, or a satisfactory combination of education and experience equivalent to two years of college work above high school.

Responsibilities

• Arrange meetings with community groups in need of health services and explain available programs and official and other community resources.
• Participate in and promote community health education activities.
• Refer individuals and groups to health specialists within a local health department or to other community resources.
• Provide ongoing individualized case management services.
• Identify health service needs among individuals and groups within minority and disadvantaged communities.
• Oversee and coordinate volunteer advocacy activities.
• Participate in planning and carry out program activities in cooperation with one or more health specialists.
• Prepare reports and participate in oral and written plans, analyses, and evaluations of health services.
• Perform other work as required.
