About The Position

Community Health Workers (CHWs) are hired and trained to address behavioral and social determinants of health through patient-centered goal setting. They reach beyond clinic walls to help patients meet their goals, creatively leveraging patient, family, and community resources to address root causes of health concerns. CHWs work to increase access to medical, behavioral health, and community services. The team aims to improve patients’ overall well-being by developing trusted relationships with patients and their families.

Requirements

• At least 1 year of experience in customer service, social service, health care, behavioral health, or a related field.
• Community Health Worker certificate, or ability to achieve certification within 18 months of employment or date of hire.

Responsibilities

• Outreach to and enroll eligible patients.
• Build trusting relationships with patients and work together to create personalized goals to improve well-being.
• Provide tailored support in accessing health care, community connections, and social support.
• Provide referrals to community social-service resources.
• Advocate for patients’ needs and offer social support.
• Communicate with the health care team about cultural concerns and barriers faced by patients in the community.
