CVS Health•Flint, MI
•$19 - $35

About The Position

The Community Health Worker (CHW) at Oak Street Health serves as a bridge between patients, their communities, and medical systems to remove barriers and promote wellness across life domains. CHWs advocate for and liaise with patients through proactive in-person and phone outreach, promoting health literacy and access to resources that support healthier lives. Success requires flexibility, problem-solving, strong communication, and an intimate knowledge of the community served. No clinical license is expected, and CHWs must work within their scope of practice. CHWs work closely with Medical Social Workers to manage patient care plans, support care-team decision-making, and coordinate clinical and complementary services. They meet patients in preferred locations, including health centers, homes, medical provider facilities, and community settings. They should expect to spend about half their time outside the center and must have reliable transportation to travel throughout the community. This is a full-time position with 40 anticipated weekly hours. The typical pay range is $18.50–$35.29 per hour. The role is eligible for a CVS Health bonus, commission, or short-term incentive program, in addition to base pay.
