About The Position

How would you like to help improve the health of your community? Achieve your goals as a Community Health Nurses providing education and clinical services to help people improve their own health. In this role with our Bureau of Community Health Systems, Southwest District, you will also work with partners in the community to make sure Department of Health (DOH) programs are available and relevant and investigate threats to public health. In this Community Health Nurse position, you will be responsible for assessing, planning, implementing, and evaluating public health services and initiatives, as well as providing direct public health services and community liaison activities. Your work will provide key support to our mission of providing infrastructure for the implementation of core public health functions in a high quality, effective, and responsive manner that will positively impact the health status of all people in Pennsylvania.

Requirements

  • Active authorization to practice as a Registered Nurse in Pennsylvania
  • Must meet the PA residency requirement
  • Must be able to perform essential job functions
  • Must pass a background investigation
  • Conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Nice To Haves

  • All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.

Responsibilities

  • Prepare and analyze county health assessments to identify public health needs and resources
  • Represent DOH on district and local health committees; help develop public health initiatives; and liaise with community partnerships to promote awareness and provide consultation, technical assistance
  • Demonstrate awareness of vulnerable populations and provide advocacy, outreach, and case management services to promote patient health and safety
  • Monitor community health programs using public health information system databases to ensure improved health status for assigned programs and populations
  • Develop and provide health education programs on a variety of public health topics to educate the public
  • Provide clinical services and consultation; set up clinics, programs; transport clinical supplies; operate and transport equipment
