About The Position

As a Community Health Nurse, your work will be focused on monitoring immunization practices, supporting providers, and promoting public health across the assigned district. In this role, you will perform the following duties: Site Monitoring: Perform visits to review provider compliance and ensure proper vaccine program standards are followed Provider Education: Teach providers and community members using approved materials to support current immunization guidance Vaccine Oversight: Implement accountability procedures with providers to maintain compliance with program requirements Community Outreach: Work with partners to support vaccination events and promote public health activities Team Coordination: Collaborate with other nurses to plan visits, share information, and support statewide immunization goals Public Health Support: Provide guidance on disease surveillance, case investigations, and emergency response when needed. Requirements

  • Active authorization to practice as a Registered Nurse in Pennsylvania.
  • If you possess an active temporary practice permit, you must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.
  • Meet the PA residency requirement.
  • Reside in or be willing to relocate to the Western Region which includes Allegheny, Armstrong, Beaver, Butler, Cambria, Fayette, Greene, Indiana, Somerset, Washington, and Westmoreland counties.
  • Be able to perform essential job functions.
  • Pass a background investigation.
  • This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
