As a Community Health Nurse, your work will be focused on monitoring immunization practices, supporting providers, and promoting public health across the assigned district. In this role, you will perform the following duties: Site Monitoring: Perform visits to review provider compliance and ensure proper vaccine program standards are followed Provider Education: Teach providers and community members using approved materials to support current immunization guidance Vaccine Oversight: Implement accountability procedures with providers to maintain compliance with program requirements Community Outreach: Work with partners to support vaccination events and promote public health activities Team Coordination: Collaborate with other nurses to plan visits, share information, and support statewide immunization goals Public Health Support: Provide guidance on disease surveillance, case investigations, and emergency response when needed.
