The work helps strengthen the delivery of essential services and ensures strong connections with local partners.

Requirements

• Active authorization to practice as a Registered Nurse in Pennsylvania.
• All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.
• Meet the PA residency requirement.
• Be able to perform essential job functions.
• Pass a background investigation.
• This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

• Evaluate county level information to identify public health needs and available resources
• Link agency programs with community activities to support local health goals
• Create public health initiatives based on community data and established guidelines
• Review information systems to track health programs and support improved outcomes
• Provide and develop education programs on public health topics to increase community awareness
• Offer clinical services and case management to promote patient health and safety
