About The Position
The Department of Health (DOH) is seeking a Community Health Nurse who is ready to make a difference in Berks County and surrounding communities. In this role, you will help assess health needs, support disease prevention efforts, and connect people with important services. This position supports public health efforts in the Southeast District by assessing community needs, providing direct services, and collaborating with local partners.
Requirements
- Active authorization to practice as a Registered Nurse in Pennsylvania
- Must meet the PA residency requirement
- Must be able to perform essential job functions
- Pass a background investigation
- Satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance
Nice To Haves
- All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.
Responsibilities
- Analyze county-level data to identify public health needs and guide education and clinical activities
- Work with community partners to link program resources and support local public health infrastructure
- Create and support public health initiatives that address identified health priorities
- Track vaccine-preventable diseases and apply surveillance systems to support improved health outcomes in the assigned area
- Develop education and outreach on important public health topics to support community wellness
- Provide immunizations, testing, and other clinical services to prevent and control disease
