About The Position

The Community Health Nurse plays a key role in supporting the health and wellness of individuals with intellectual and developmental disabilities residing in community homes regulated by Pennsylvania’s 6400 and 6100 regulations. This position ensures compliance with all applicable regulatory standards and provides clinical oversight, staff training, and ongoing health monitoring.

Requirements

  • Skilled in the use of feeding pumps and administration of parenteral nutrition and medications through G-tube, J-tube, and GJ-tube.
  • Comfortable with and willing to receive training on Tracheostomy and Ventilator dependent clients.
  • Excellent clinical assessment, critical thinking, and communication skills.
  • Proficient in Microsoft Office and electronic health record systems.
  • Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the Commonwealth of Pennsylvania (required).
  • Minimum 2 years of nursing experience, preferably in community health, home health, or intellectual/developmental disability services.
  • Must always maintain BLS certification.
  • Must be able to work flexible hours to meet the needs of WCS and the residents.
  • Good interpersonal and communication skills required.
  • Must possess a current/valid Driver’s License, an acceptable Motor Vehicle Record (MVR) as mandated by Woods’ insurance company, and successfully complete SMITH driver training.
  • Must be able to lift a minimum of 50 pounds, be agile, physically pursue clients on foot, physically restrain, bend, squat, stretch, reach, push up and stand for several hours at a time.

Responsibilities

  • Ensure all health-related services are in compliance with PA DHS Chapter 6400 (Community Homes) and 6100 (Services for Individuals with Intellectual Disabilities).
  • Complete and maintain Lifetime Medical Summaries for all individuals as required under 6400 regulations.
  • Ensure documentation of health services is accurate, timely, and accessible in accordance with licensing requirements.
  • Conduct and complete weekly nursing assessments for individuals in assigned homes.
  • Perform regular rounds to residential sites to assess overall health status, infection control practices, and safety conditions.
  • Prepare and distribute weekly nursing reports to the interdisciplinary care team.
  • Participate in 90-day medication reviews with the prescribing provider and care team.
  • Monitor for any acute or chronic health changes and ensure timely provider follow-up.
  • Provide initial and ongoing training to direct support staff on health-related topics including medication administration, infection control, seizure response, diabetes care, and more.
  • Provide on-site coaching during rounds or when concerns arise related to health services.
  • Support new staff orientation and competency checks for medication administration.
  • HCSIS point person for reportable medical incidents
  • Conduct monthly Medication Administration Record (MAR) audits to ensure accuracy and compliance.
  • Perform medication, pass observations as needed and provide feedback to staff to ensure safe medication practices.
  • Complete prior authorizations and medication refill requests with providers and pharmacies.
  • Ensure all individuals have appropriate medication equipment and supplies (e.g., glucometers, inhalers, and PRN meds).
  • Regular communication with Medical Director, Director of Nursing, House Supervisors, and other interdisciplinary team members.
  • Attend care planning meetings, hospital discharge meetings, and other relevant team discussions.
  • Serve as a clinical resource to residential staff and families regarding health conditions and treatment plans.
  • Maintain clear, timely, and complete documentation in the agency’s electronic health record system.
  • Track and follow up on medical appointments, labs, and specialist visits.
  • Ensure that consultation forms and post-appointment notes are completed and reviewed.
  • Collaborate with case managers to ensure families are notified of a resident’s ongoing health status, appointments, and changes to conditions.
