About The Position

The Community Health Navigator conducts outreach and enrolls members in Reema's care model, then builds and manages a caseload of 80–120 members. The role assesses member needs, including social determinants of health, and develops member-centered care plans. Navigators build trusting relationships, help members work toward health goals, coordinate referrals and appointments, address barriers to care, and connect members with community and health resources.
