About The Position

The Community Health Navigator conducts phone and in-person outreach to enroll members in Reema's care model, builds and manages a caseload of 80–120 members, and provides coverage for other Guides as needed. The Navigator assesses member needs, including social determinants of health, and develops member-centered care plans. The role focuses on building trusting relationships and helping members work toward their goals by addressing barriers to care, coordinating referrals, and supporting appointment scheduling and attendance. Navigators use phone, text, email, and in-person visits to engage members, connect them with health and community resources, and accompany or transport them as needed. They organize their work independently to meet daily metrics and monthly goals, document member interactions and current care plans in the Reema App, and maintain confidentiality and professional boundaries. This is remote, independent work in a fast-paced startup environment, with occasional required in-person team meetings. The role requires collaboration with a multidisciplinary team, adaptability as priorities and tools change, and the ability to work with members experiencing significant hardship. Responsibilities may evolve as business needs change. Candidates must be at least 18, available Monday through Friday during core business hours, use technology-based tools daily, and be able to lift and carry up to 25 pounds occasionally. Offers are contingent on passing a pre-employment drug screening and background check. For field-based work, a valid driver's license, insurance, and reliable transportation are required.
